Failure To Include All Attachments And Meet All Guidelines.
Will Result In Immediate Disqualification.



The Fishtown Athletic Club Alumni Accociation (Herein "Alumni Association") will allocate funds available (for 2009) and annually for scholarships as described below, provided that it has the funds to do so.

A child is eligble to apply for the scholarship if he or she meets the following requirements:

"EXTENSION OF ANNUAL SCHOLARSHIP AWARD ELIGIBILITY BEYOND CURRENT FISHTOWN ATHLETIC CLUB PLAYERS"

All regular "non-sponsor memebrs" of the Fishtown A.C. Alumni Association, Whose status is that of "Good Standing" as of March 24th in any given year, may have any qualifying children in their immediate family apply for the Alumni Association's Annual Scholarship Award. Except as amended herein, all previous rules and scholarship award guidelines shall apply.



AMENDMENT #3
FISHTOWN ALUMNI SCHOLARSHIP AWARD
JANUARY 16, 2002


"EXTENSION OF ANNUAL SCHOLARSHIP AWARD ELIGIBILITY COME TO THOSE CHILDREN LIVING IN THE 19125 ZIP CODE"

For the first time in the history of the Fishtown A.C. Alumni. Children that never played with the Fishtown A.C. can now apply for the Alumni's yearly scholarship awards. As long as they can fill all of the Alumni's guidelines. Any child living within the Citie's 19125 zip code. May request a application from June 1st, until June 30, each year. This will open the Alumni's scholarship awards to hundreds more of deserving children. These childern must get letters from their teachers and or the principal of their schools.

PLEASE NOTE:
The applicant must submit full proof of living in the 19125 zip code if and when requested.






Follow all Guidelines below other then number #1 (Being a player for the Fishtown A.C.)

Being a Child of any active Alumni member (May live any were in the U.S.A.)

Children playing with Fishtown A.C. may live in any zip code. And most follow and fill in all guidlines



UP DATED GUIDELINES

1. The child has been a member of the Fishtown Athletic Club during the past twelve months prior to the submission of an application for scholarship.

1A.~ Or if the child lives in the 19125 zip code

2. The child has maintained a minimum grade point average of 85 out of 100* (or its equivalent) for the two years immediately preceding his or her application for a scholarship.


2A -UP-DATE ~ March 15, 2006
Starting as of June 1, 2006
After months of hashing and re-hashing on the subject, the FAC Alumni staff has voted to approve the lowering of the minimum grade on the scholarship applications from 90 to 85. The results of the staff voting was 4 in favor and 2 opposed. Therefore, due to the majority vote, the proposal has been approved and will become an amendment to the FAC Alumni scholarship application process. Note: The grades must be a minimum of 85 in EACH of the last 2 school years. If either year is below 85, then the applicant has NOT met the new guidelines.

3. The child produces a character refernce on letterhead paper from the principal or head of his or her present school, one of his or her present teachers, or a coach for a team on which he or she is playing or has played. The character reference must state that to the best knowledge of the person making the reference, the child is drug-free and has not been adjudged delinquent.

PLEASE Take note:We have had a number of applications disqualified. Because the person (coach, teacher exc.) writing a letter on behalf of the child was never told that the "Drug-Free" was to be apart of his or her letter.

4. The child is a student in or has accepted an offer to attend an accredited private high school, parochial high school, university, or trade school and seeks the scholship to cover part of the tuition for the accredited private high school, parochial, college, university, or trade school during the next school year.








Guidelines up Graded Amendment #1 November 13, 1996 ~ Students entering grades seven (7) and up, including high school students and college students are welcome to apply long as they carried a 85 average the last two academic years. This makes two additional classes eligible to apply.

5. The child and his/her parents or guardians have the responsibility to obtain the documents necessary to show that the child meets the minimum requirements described above.

The Alumni Association intends the scholarship in a given year to be of equal amounts. Thus, the Alumni Association will divide the funds available by the number of applicants. And can give as many as five awards a year. The Alumni Association deems qualified to receive scholarships and the quotient becomes the amount of the scholarship for each successful applicant for that year.

6. The Alumni Association will pay the scholarship directly to the school or reimburse the parents or guardians of the qualified child upon proof that the parents or guardians have paid the tuition to the school.

7. Note: Photocopies of:

1. - Last Year and This Years Final Report Cards must be provided.

2. - Any other grade supporting docments (awards received) maybe sent along with the application.

3. - Please mark the grade and year over top of each years final report card.(on photocopy)






June 1 And must be returned before June 28th.

Effective June 1, 2009, ALL Scholarship Applications must be Post Marked between June 1st and June 28th. Any mailings Post Marked AFTER June 28, will NOT be accepted.

Most Children put this part on a separate page. This helps the judges to understand this student better.

DID YOU DO EVERYTHING RIGHT?
The last thing the alumni judges wants to do is to disqualify an application of a great student that really deserves a fair chance in everyway to win.

But when time is not taken to follow the guidelines in full detail. There is no other way then to disqualify that application.

PLEASE if you think you may have over looked any-thing? Just go back and check it again its well worth the time to make it right.......................

Every year we have to sadly disqualify one or two applications because of character reference letters (person writing the letter leaves out) that to the best of his/her knowledge the child is drug-free and has not been adjudged delinquent. Ask the letter writer to please remember not to leave this out. (Show them the guidelines page. Section #3 we sent you)

Or a Report Card Missing? ~ Remember to make copys and send in the last two years final report cards.


Remember if you don't win a scholarship this year there is always next year don't ever give up we are all pulling for you!

We have dedicated this plaque to all of those outstanding people from the Monday Night Early Leagues bowlers that was the back bone and the heart and soul the real spirit of our Scholarship Fund from Monday night September 12, 1992 until Monday night December 20, 1999 these wonderful people form every neighborhood in the City took $52,000 in 50-50 chances. Yes that is right $26,000 of this money went into starting our Scholarship Fund. Please we invite you to stop by the desk to see and read our plaque with the names of all our award winners from each year. And realize that these Children are getting the chance to get another year of schooling because of you!
